Diagnosis of Sexually Transmitted Infections in Older Adults - #85
Take QuizDiagnosis of sexually transmitted infections in an older adult.
Presentation in Men | Presentation in Women | Primary stage | Secondary stage | Tertiary stage | Latent stage | Neurosyphilis stage | Diagnosis | |
Chlamydial infections |
Painful urination Mucoid/watery urethral discharge Painful/swollen testicles |
Painful/burning sensation when urinating Increased vaginal discharge or bleeding Abdominal/pelvic pain |
Nucleic acid amplification test (NAAT)-preferred Microscopy, culture, antigen detection, genetic probe methods Signs and symptoms (can be asymptomatic) |
|||||
Gonococcal infections |
Burning sensation when urinating White, yellow, or green discharge from the penis Painful or swollen testicles |
Painful or burning sensation when urinating Increased vaginal discharge Bleeding |
Nucleic acid amplification test (NAAT)* Microscopy, culture, antigen detection, genetic probe methods Signs and symptoms (can be asymptomatic) |
|||||
Syphilis |
1 or more chancres at site of exposure around 3 weeks after infection (range of 10-90 days) Patient highly infectious Chancres can heal on their own, even without treatment |
Mucocutaneous lesions can occur as chancres fade around 6 weeks after infection (range of 3 weeks – 6 months) Rashes may first appear of palms of hands or feet, then on other areas of the body Patient may experience lymphadenopathy & constitutional symptoms as well as tongue lesions Patient highly infectious, especially if there is direct contact with a moist lesion |
Manifestations in skin, bones, and cardiovascular system Patient is not infectious |
Early Patient has reactive nontreponemal and treponemal tests within 1 year of onset of infection but no symptoms Patient potentially infectious Late Patient has reactive nontreponemal and treponemal tests ≥ 1 year of onset of infection but no symptoms Patient is not infectious |
May occur at any stage Any patient with syphilis and neurologic, ophthalmologic, or audiologic symptoms need a neurological exam and CSF evaluation via a lumbar puncture |
Nontreponemal tests (RPR, VDRL, TRUST) and Treponemal tests (FTA-ABS, MHA-TP, TPPA, TP-EIA, CIA) Examination of cerebrospinal fluid to definitively diagnose neurosyphilis |
Diagnosis: N. gonorrhoeae not only causes similar clinical syndromes as C. trachomatis but also coexists in a significant proportion of patients with chlamydial infection. Therefore, any testing for C. trachomatis should also prompt testing for N. gonorrhoeae2
Sexually Transmitted Infections (STIs) are increasing among older adults. These infections may include chlamydia, gonococcal infections, and syphilis5. Patients may experience no symptoms or symptoms may be mistaken for a urinary tract infection.
List typical presentations of infection (in men and women) presenting with sexually transmitted infections and subsequent diagnostic testing.
In 2017 there were 1.7 million cases of chlamydia (a 22% increase since 2013), 555,608 cases of gonorrhea (67% increase since 2013), and 30,644 cases of syphilis (76% increase since 2013)6 in the United States.
Science Principles
Chlamydial Infections: Caused by Chlamydia trachomatis
Gonnococcal Infections: Caused by Neisseria gonorrhoeae
Syphilis: Caused by Teponema pallidum
- List common symptom presentations associated with sexually transmitted diseases both in women and men.
- List appropriate testing to identify chlamydial, gonococcal and syphilitic infections.
